Person-Centred Care and Care Integration

This Special Issue of the Journal of Integrated Care will identify examples of best practice, and examine how to enhance existing care delivery methods to achieve person-centred care. It will identify evidence including barriers and facilitors of person-centred care in Integrated Care programmes.

Submissions are being sought that cover any of the following themes:

• Best practice in person centred care and practice and Integrated Care
• Emerging research and current evidence base
• Educational and training of person centred care and practice in Integrated Care programmes
• Reflection on the importance of the person and hearing their voice in person centred care and practice in Integrated Care
• Tensions and conflicts within person centred practice or Integrated Care

NB: The following topics are indicative and not exhaustive.
